I have rarely purchased a complete bike, typically a track bike. I usually trade in framesets.

The frameset that was damaged had no fork, nothing loose in the box. UPS managed to drop a sharp object on a seat stay which was unfortunately a composite which is not designed for impact strength. In the end I got most of my money back, but not the amount the bike was insured for. Probably should have tried the lawyerly route, but that's not really my approach. UPS's policies being what they are I'm lucky I got the money I did.

On the constructive side:

1 - Yes, double box. It works.

2 - Insist on use of dropout inserts.

3 - The foam insulation tubes work.

4 - Filling the loose space with "peanuts" prevents penetration of blunt objects.

5 - I've even considered lining the sides with 0.125" plywood (akin to the idea of using wood blocks in the corners.)

The last bike I received also had a beat box, but the frame (a Lotus 110, carbon fiber monocoque aero frame, a'la Chris Boardman) was lovingly packed. It took about half an hour to unpack, must have taken over an hour to pack.
