It's worse than tire sizes, nobody really knows precisely... but roughly 45 should be size 10. I know that Nike's run small for size (got a pair of 43's SPD Gabuche's {for sale; 3 yrs old, black with blue inserts, slightly used, includes SPD cleats, $25 shipped} that are smaller than my Shimano 42's) than other brands.

Since I damaged my right foot, I've found that it's about 1 size larger than my left!
