Dare I add that if one uses the forbidden (small cog) + (small chainwheel) combination, a tinkling sound can be generated because the chain is intermittently tickled by the teeth of the large chainwheel as it goes through the front derailleur cage?
The sound intensifies under load because in such a case the chain abandons its graceful 's' bend and tries to make a strait line between gears, thus causing greater overlap with the large chainwheel.
I don't have to worry about it on my 5 speed, but my modern up-to-the-minute 7-speed does it; sometimes even on the 6th cog.
