I could see this happening if one attempted to use it at the local BMX freestyle track or if I was working on it while imbibing in my favorite brew and cross threaded an expensive bottom bracket fixed cup. I'd cry first then anger would set in and then the ballpeen hammer would fall repeatedly against the down tube. Just my guesses. I really don't know.
