We used threaded forks and quill stems that allowed reasonable bar height, that's how.

Perhaps I'm the exception, but I've never had any problem getting an optima l position or lacking standover clearance on a traditional non-extended headtube frame. And they, to my eyes, look heinous. How did we cope for 100+ years without them?!?
