Don I agree with Bob Schenker that it may be a DIY project, but it would have taken a bit of skill. The 5 bolt 2" (50.8mm) BCD boss is offset quite a way inward from the normal position. A bit beyond the capabilities of most tinkerers. The grey paint in this area and the squared off ends of the outer chainring arms do suggest DIY.

I would expect most manufacturers would have done as Nervar did for a triple. Mount the outer ring to a standard double arm and add the 3rd ring further inboard, rather than make a different right arm for triples.

However mounting the middle ring to the arm reduces the possibility of flex due to chain tension when on the inner ring. As thin as steel rings were, I can see why someone (whether tinkerer or manufacturer) might think this sufficient reason to do it "the hard way".
