OMPAX was a private label trade name used by an importer well into the bike boom of the '70's. I had been told that the importer was from Australia but the maple leaf logo suggests Canada. Probably a container by container operation with a variety of manufacturers and assemblers used to meet the specs of the importer.

Kuwahara making OMPAX label bikes? It wouldn't surprise me, but the ones I have seen were made by Tsunoda.

OMPAX was just one of dozens of Japanese private label bikes from that time period.
