What was the original headset? The Dura-Ace EX used 3/16" balls in Campy-compatible retainers, but the cups had a sort of labyrinth seal built in which made it taller stack-wise than many other contemporary headsets.

Make sure that you're using the proper size balls, and that the retainers (if any) are properly oriented. You may need to substitute a unit with shorter stack height.
