I like to clamp the pad holder in a vise, I use aluminum angle in the vise jaws for gripping the threaded post, and push the old pad out the open end with a big flat screwdriver. Put a teeny, tiny amount of grease or Vaseline on the leading 1/8" or so of the new pad and slide it into place, then stand the pad end on the work surface and push the holder down onto it with the heel of your hand. Minimal force needed, has always worked for me.
