Craig, I am far from an expert on anything but I have installed several of these headsets. Thread the cup onto the fork threads until you seat the bearings. Now back off a 1/4 turn and slide the toothed washer down, locking the teeth to the top of the cup. Then you need to slide a locking washer down on top of the tooth-washer. Depending on the bike, you need either a flat sided or "pronged" lock washer. Then screw down the locking nut. The locking washer should keep the cup from turning when you tighten the lock nut. If the locking washer begins to slip, then use a pair of large chanel locks-protecting the cup with a rag or something and turn the nut, not the cup. If you are using a centerpull brake cable stop, some have an integrated tange to act as the locking washer. This should do the trick. If you are still having trouble, clean the threads good and wrap some teflon tape around them before screwing down the nut. As always, preloading the bearings is hit and miss. Use Grade 25 bearings, don't over pack and not too tight as too induce indexing. Oh, if this is a headset that already indexes and it has the "V" shaped races, try the next size larger bearing on the lower race. I've done it and it works fine. Those headsets are sometimes hard to find in the "right" size so improvising may help you retain the original. Our local builder/shop owner/racer, Glenn Swan, turned down my pitted race on his lathe (in the winter when he was not busy). Good Luck. I hope I was able to help.
