I think that's the Bullseye. I believe saw one in Mechanics Illustrated when I was about ten years old, and I have a picture of one in a book around here somewhere. Sloane's, maybe? If I understand the intent of the design, I think yours should be rotated two-fifths of the way around counter clockwise, when viewed from the drive side, but I might be dead wrong about that.
