Did it ever do this before, or is this a new chain/freewheel?

I assume the chain isn't sagging when you get on the 13 toother. If the derailer still has some wrap capacity on the small cog, the chain isn't too long.

Check for a chipped tooth on the 13, or a stiff link. Perhaps a semi stiff link only kinks up when the chain has to wrap the small cog in the rear.

If no, check the chain line. Do the front chainrings center on the freewheel? If they're off, you may need to change the BB spindle.

Is the skipping only on the small chainring 13tooth combo, ie the "no-no" combo? If yes, how long are your chainstays? If around 16 inches or less, the frame may be too short to accomodate the small-small crossover.
