Hilary already addressed the topics of fixed gear, derailleurs, and front changers.

Judging by photographic evidence, rear derailleurs started appearing at Paris-Roubaix by about 1933, and by 1934 most riders seem to have had them. Pictures from 1936 P-R show the Super Champion system in use, with the tension pulley behind and below the chainring.

In terms of racing bikes, high flange hubs have been around in various designs since at least the 1930s (Paramount hubs are one example). But... examining my old racing photos, it seems that high flange began appearing in the European pro ranks by about 1954, and remained common until about 1966.
