I've seen several Campy hub flange failures. Two on my own rear low-flange record hubs, drive side. My Miche hub failed in a big ugly bulge, without cracking. A friend's C-rec large flange failed, actually the front, as I recall. It's funny because those hubs are so heavy; I guess they just weren't heavy in the right places.
I ascribe the failures that I had to over-tensioning of the spokes which, in hindsight, I used to do routinely. Campy made good on the first failure before they became stingy on the warranty stuff.
One tip that may help avoid flage failures is to always loosen your spokes before cutting a hub out of an old wheel. This is already a must, if you hope to resue the rim.
