Woah, that's a rare bird. I think that Campy had a four-hole SR depicted in the Olympic catalog as a drawing, but in the photo at the front I think it's a three holer. I've always assumed that it was an early drawing of something that never met production. As for this derailleur being first-generation Super Record, I'm not so sure. I think the last year for pre-CPSC fronts was 1977. So the no-holes with CPSC lip derailleur didn't come out until 1978, which was the last year for the first gen. rear derailleur (unless they kept making those along side the second gen. model.) Anyway, I think that they made the no-hole CPSC compliant fronts for a couple of years before introducing the four holer, which apparently they did as a Super and as a Nuovo. In any case, this puts us a year or two into second generation rears. I have no doubt that your bike was equipped as you say, but I suspect that the front and rear derailleurs were made at slightly different times.

Of course I could be wrong. That's why I've detailed my line of reasoning. If anyone has any information that conflicts with this perhaps we could apply it to refining the Chuck Schmidt timeline. The whole pre-CPSC/CPSC, first-gen/second/gen, holes/no holes era is pretty confusing on the front derailleur issue.
