Wasn't it a Hooker that more or less made Rebecca Twigg lose the first-ever women's World Timetrial Championship when she was the heavy favorite? Chain jumped off twice, seatpost slid down in frame, finally the stem came loose and she gave up, if I remember correctly. Probably outside our timeframe.
