You can use an old simplex or Campagnolo pushrod model and file the rod to skew the cage slightly. That is what I did on my Bob Jackson tandem before I switched to front drive. The other alternative is to fit a braze-on shifter with the best being the French-mounting style that required one single water-bottle boss.
