That shifter (Part No. 1013/1b) goes with (vis. red motif) the Cambio Gran Tursismo (AKA "GT", AKA "The Steel Beast" ;-). It appears on Page 4 of the Supplement to Catalog No. 16. And, not inappropriately, it is pictured between the GT rear derailleur and the aforementioned Elefante shifter.
