I think your foot's width comes into play for proper sizing. I wear a US 10.5 D-width, and size 45 Dettos feel just right. I have a pair of 44.5 Sidis that I've used for years, but they are probably a tad tighter than I'd buy today, but 45 would be fine for me in Sidi shoes also I think.
Baron and Marie-Cecile at Renaissance may still have some NOS Dettos as well - I bought one pair each of the Art. 7 "drilled" Coppi and the Art. 2000 wood-soled Superlight from them recently (in size 45 - you don't get a chance like that very often IMO), and they are very nice.
