This is usually indicative of the screw bottoming out before the proper friction is reached, because it should theoretically be possible to tighten the d-ring such that you can't even move the lever. I would suggest grinding down the screw ever so slightly and retightening. It might also be that paint or other gunk is inside the braze-on. I saw this very often on repainted bikes.
