Steve (wink), chuck, tom, et all,

I think if a 1R stem has a holding problem, it could be because the wedge that pushed against the bar could be broken, and this piece should be checked. Given what we bikeshop service monkeys see (and do) it doesn't surprise me that this could happen when the pieces aren't lubed, or assembled carefully. I believe, like Chuck is suggesting, that if it's lubed, and the correct torque is applied, there shouldn't be a problem.

Many people say that the 1R all were doomed to slip. I believe this is a fallacy. I have been using one on my tandem for over 20 years and it has never slipped or creaked. I used them on a number of other bikes too, always with the same results. When I installed the stems, I always made sure everything was thoroughly lathered in grease and it worked. I did however have one break across the front, likely because the grease allowed me to overtighten the stem (Cinelli promptly replaced it free of charge).
