I find the easiest way is to shear off the inside of the rivet - I use a drift made from an old piece of frame tube; give it a nice clean edge; insert it into the headtube until it contacts the rivet, making sure the drift is hard against the inside of the headtube; whack it with a hammer and the peened over part of the rivet will shear off; then a matter of finding something to poke it out from inside.
