I don't know how accurate your 21.9 measurement is, but that is very close to the nominal 22.0 French stem diameter. Seems odd you would find that on a Hetchins, but one never knows. Anyone know if smaller steerer ID's were common on British bikes in the 50's? Perhaps some of the old steel stems had a smaller diameter. I'd try a French stem, or a 22.0 Cinelli, which were made for French bikes. But you can easily sand down a 22.2 Cinelli, although as you say, it doesn't do the finish any good.
