Could you fabricate a bracket that rises vertically from the brake center bolt? I've pondered this solution to similar problems. Have not executed it, though; no time it seems to experiment.

If the headset has the typical french sawtooth washer arrangement, you might get ~5 mm of threads back by switching the top races to something like campy that uses plain washers, and using the mafac hanger as the sole washer. If you're willing to use off-topic parts, you can get a hanger that clamps to the stem quill.
