David,

You simply screw the injector onto the back of the freewheel, and then inject grease (either through a dedicated grease port, or screwed directly into the body of the injector--Phil Wood tubes fit perfectly), and just wait for the old grease to come out the other side of the freewheel. Simple, and very effective.
