One possible cause of flats at the valve is the little lock ring on the valve.
I used to get flats at the valve stem occasionally until I figured out that I was overtightening the knurled lock ring that threads around the stem and is meant to lay flat against the rim. Since then I just tighten them enough so they don't rattle, or I don't use them at all.
