Well, I don't now how it happens, but it does happen. I've had pedals unscrew on me twice, both times on a (different) fixed gear. Both times the outermost crank threads were damaged enough that you couldn't get a pedal to thread in, but after running a tap through it, backwards as was suggested by another poster, they have been fine ever since. This time, of course, using a big wrench to tighten the pedals down.
