I have had it happen to me twice recently...first as a buyer and then when I was selling an item.
Turned out the reason it was not an instant transfer, in both cases, was that the credit card used as a back-up had expired. Once I updated my credit card information, the next payment I made went through as an instant transfer.
The buyer was concerned that there was something on my account that was keeping him from doing an instant transfer, but when I related my experience to him he discovered his cc had expired also.
