Is there any truth to the story I have heard that the etched and/or the engraved versions of Campy crank arms broke due stress fractures from these processes?
What are the locations of the logos on engraved and etched crank arms?

It is possible for arms to fail in fatigue at the stamped logo, but not the laser-etched ones, AFAIK. I've seen one picture of one arm (a Stronglight) that broke that way. As always, see Ric Hjertberg's page for some enlightening broken crank pics:
