That is a cut out area for optional aero cable routing. Cobalto levers and maybe some Delta's allowed for both aero and standard cable routing. The protocol for aero routing was to file out the area shown in the photo, install a cable guide in the lever and install an end plug in the hood. For a number of years in the 80's this was the only way to get a Campy aero lever
