1) That's easy: no one. At least not in the traditional sense of "source." There are none left at any Distributor of note. None has been made (legally) in a very long time. 2) Kurt: the simple answer is no for black, yes for white (shield logo), but since anyone could have bought the black GS ones to replace your worn-out gum R/SR ones, I'm not sure it's a big deal if someone wants to use black hoods (of either logo) on R/SR levers.
