Peter,
Rather than blame the stem, I would step back and look at the "big picture" - after so much grief about Italian bikes - maybe the bike is telling you something. There could be a rider/bicycle incombatability issue here. I would get rid of that Cinelli ASAP before we're all saying "she done him in".
Just checked - the Cinelli stems on my bikes are all purring.

Maybe this was indeed a replacement pinchbolt but I swear I've seen this on other Cinelli stems offered on eBay. I believe this style is rarer than the later one but until this happened I must admit I never gave Cinelli stems much thought! Now whenever I ride with one, I shall contemplate with no small measure of fear and dread! And this from a guy who has been happy to ride his PX-10 with an AVA "death stem"!
