Evidently a practice that preceded the charting in the Fit Kit, but certainly one that took into consideration variables in bodily dimensions was a simple measurement that amounts to an individualized "cockpit" designation: the measurement for comfort fitting the stem length was a simple function of top tube length and saddle placement (the line of the seat tube simply bisected the saddle) and was measured in Anglo-Saxon fashion by using a body part, by placing the elbow against the nose of the saddle and holding the hand in line with the forearm and fully extended, the longest finger should just touch the handlebar. There you have it, circa 1970, use a 1.09 multiplier for the inseam to derive saddle height, and practice the Olde English elbow to finger tip measure to finish placement of the handlebar, and you'll have vintage placement, warts and all.
