What you probably have is a freewheel tool made by Campagnolo to fit Regina frewheels. This tool was included with the Campy tool cases for many years before Campy came out with their own freewheel. The tool was later modified to fit Campy freewheels by changing the insert piece. Unfortunately, I've never seen this piece available separately -- you have to buy the whole tool, if you can find it.

An acceptable alternative is the Bicycle Research tool #CT-9:

Interestingly, when I bought my Campy tool kit in 1984, it still included the old Regina-type freewheel tool even though the Campy freewheel had been on the market for several years already.
