"Maitland" was a pattern of bars sold by GB in the mid 1950's, available in chrome or alloy. They were 15.5" wide, with a 5.75" drop and a 2.125" forward extension. Can't help with the stem, other than that Titan pattern stems were made with other names on them. I have one with "Douglas" instead of Titan, but in the same script and pattern of stem as Titan, and I've heard of others.
