I was surprised to see the variety in these too. When I worked at a bike store in Chicago in the late 70's, we had a bunch of these, and we liked to put them on our QR levers. This was in the days when everyone had just rediscovered Benotto tape, and there were some very fancy multi-color wrapping schemes, not to mention all those colored AME lever hoods, and multiple colors of cable housing. We had multiple colors of the shift lever covers, but I only recall one shape and size. I believe ours were cuffed.

I had yellow ones on the QRs of a nice set of wheels. At some point I decided that they were just too disco. Attacking the front wheel, I was unable to twist the cover free, so I cut it off. Apparently I was immediately remorseful, since I left the remaining one in place. This is why I am looking for a single yellow.

As penance, I have since decided that I am only going to own bikes with yellow accents.

http://bulgier.net/pics/bike/Campy/RubberShifterCovers.jpg shows the different styles I found in my junk pile just now. Note some have "cuffs" and some don't; also note the different sizes.

Assuming the oldest ones had an open-C typeface, these must all be newer, so there are probably still more styles out there (one more at least).

Anyone have anything like a timeline for these?
