I think this may be some sort of chain tensioning system for old BSA fixed geared bicycles or possibly an old BSA moped/motor-bike.
Campagnolo made a Tendi Catena (spring loaded chain tensioner with one jockey wheel.) It is shown in Campagnolo's very early catalogs and if I am not mistaken states that it (the Tendi Catena) is for use with old mopeds/motor-bikes.
