The crankset in question looks like a creative dyi job to me. The outer chainring is bolted to the outside of the spider and appears to be spaced out to make the normal outer ring into a middle one.

I remember in the late '60s - early '70s, going on one our annual snow camping trips around New Years in Yosemite. It was a dark, oppressive, cold day with piles of snow and ice layered all over the landscape and the road. A guy in street clothes and a parka was riding up from El Portal on a Schwinn Varsity laden with army surplus duffel bags on a Pletscher rack. I've never forgotten that image.
