Obviously this is a great source of irritation. There's probably a logical explanation, such as the head lugs were another worker's responsibility who never got the job done and the frame went to painting the next day. Clearly a lapse in quality control. I have a similar flaw on my Nishiki Ultimate frame where the "gusset" on the top head lug could have been filed out, a minute's work, but was left untouched, and it irks me every time I look at it. I could see something like that being left on a frame produced in the thousands each year, but this was a limited production high end model.

I'd say go ahead and fix it before repainting if you are up to it, and it appears that you can master these things.
