Mark and All,
I once did use a longer bolt for the clamp when I set up a tandem with bar ends for a third brake. I didn't like the Mafac lever pulling both front and rear brakes with the drum on the other, so I set the drum up with a mountain bike friction thumb shifter to use as a drag brake. I needed the longer bolt, and a pair of pliers to flatten the dogs of the clamp parallel to get the bolt secured. But it worked well. That was for the sleeved bar right next to the stem, so you can get away with just about anything for a Sturmey shifter if you're willing to bend a bit.

...> It still uses the original fixing screw, but just barely
There seems to be some variation in clamp length as someone else was able to do this. But I'm sure my clamp isn't long enough to use a normal fixing screw with my bars. But off list, Ted Ernst suggested a longer, curved bolt.
