The bolt holes on my Campy GS 3-Arm crankset are 9mm in Diameter. Its an odd size today, but may have been popular in the 60's.

A friend's Nervar 3-Arm Aluminum Crankset uses the same size bolts, as do TA Pro 3 cranks. see: http://www.blackbirdsf.org/ta/pro3.html TA Pro 3 bolts (ref:43) are still available ... Bicycle Specialists in Canada has them.

My GS has a "step" only on the outside of the crank arm spiders (as with Pista cranks). Because of this, the inner ring has to be attached to the outer. In Campy's case, I've only ever seen 52/42. Fortunately, Campy used the old 116mm BCD standard, so other manufacturer's chainrings fit. I have a 52/36 Cyclo mounted on mine.

With some effort all kinds of mixing and matching can be done with 116 BCD cranks and chainrings. TA made an adapter for 3-Arm cranks to TA 6 pin chainrings and to other patterns. The possibilities are endless, making this style of crank arm almost as versatile as the 49D.
