I asked Josh Lehman Bicycle planner for the state of MA about bicycle planning in his state and all states. His reply is below ''Hi Charles -

MA passed a law in 1996 requiring that all State road and bridge projects incorporate access for bicyclists and walkers (except in certain situations, such as freeways). The law has been strengthened with various engineering directives and design manuals over time. We recommend but do not require that localities conform to our design requirements. Some localities, such as Cambridge, have stringent requirements for incorporating bicyclists and walkers into their own projects.

Oregon has pretty firm requirements for including bicyclists and walkers; WA and FL as well. My guess is that the best place to get a handle on which states require what is to check the federal Pedestrian-Bicycle Information Center website, http://www.bicyclinginfo.org ''

Josh Lehman'' Josh went from bicycle coordinator for bicycle routes for City of Seattle in 70's to Wash. DC Federal government same type of position and now planning for state of MA. A person we all should be thankful for his dictation and expertise on our behalf.
