A couple of points to add: Please see if your chain is to worn or stretched. This could cause the chain to pop forward over a tooth under pressure. Maybe a newer chain would cure it. Unless your 13 is worn too much and the chain is newer. Then the same event occurs. Every once in a while the width of the side plate of the chain is such that the chain bottoms out at the base of the teeth onto the shoulder of the cluster. Then the chain doesn't seat properly and does not get the full tooth bite and also pops over and forward. We saw this quite a ew years ago when the transitions were happening in the bike game from friction to index and chains were being redesigned. We looked at it very carefully and finally realized the imcompatability in some cluster-chain combos. Devious and uncommon, but a definite contender along with other hints you're
getting.
