The discussion seems to have pretty well covered the frame.
I would give the fork steerer a good look over, maybe a check the fork
in a fixture and a have a magnaflux job done by a machine shop.
It may be the angle of the picture, but the fork crown seems to not be
parallel to the headset cup.
