Hi Charles after you mentioned the forks might not tie up with the frame I removed them and checked for any marking on the steerer. It appears that the forks and frame are definitely a pair as the BB shell is stamped 65 and the steerer of the forks is stamped with a very clear 6? with the ? looking as though it could be a 5.
So I guess I don't know why the lugs don't look like those you have come across before, perhaps an aberrration, a special request for those lugs, just plain strange?
What made me wonder about the frame repair is that there doesn't appear to be any evidence of a respray. For instance in places where it has been scratched, or places you might expect overspray (steerer, inside ST, BB etc). Perhaps it was totally shot blasted before being refinished though.

"what Dale said. In addition, it appears the fork is an early-70s item, the main frame might be contemporaneous, but those are lugs you never see on any colnago. Could this be some kind of price-point frame, like the International? I know we here in the US didn't see all the flavors of Colnago Ernesto was pumping out in the '70s. Could this be a shop-brand bike, for instance?
Interesting anyway. Too bad about what appears to be a lug-catastrophe on the down-tube..
