I have one that clamps to the steer-tube only, it is only about 6mm thick and has an M4 bolt that clamps it tight after it is screwed on.
I have never seen one that clamps to both except for ones that are part
of the gooseneck like this one by bike friday that I've attached a
picture of, it is welded onto the "goose-neck" and is a skirt that comes
down around the top of the steer-tube, clamping onto the top of the
steer-tube. I think a few other folding bike types things use a
similar thing, but I do not remember off the top of my head...
I know the picture will not come through to the list, but it will go
to Peter, so I attached it anyway, if anyone needs to see it, I can
resend to individuals... I was looking for a picture of it on their
web-site to link to and found this, which might be close to what Peter
was asking for...
