Are you talking about the wedge that holds the handlebars from rotating in the stem? If so, you might want to check for cracking under the handlebars. The stem has a very weak line across the bottom towards the rear. I had one loosen up and then crack completely. Fortunately, I was able to ride home, gingerly.
