I had a similar issue when rebuilding a Bruce Gordon frameset with a
new headset after a repaint. Turns out the top nut was bottoming
(topping?) out on the steerer tube - the new headset
had a slightly shorter stack height. I added a washer and was then
able to tighten top nut and cup as necessary.
