I'm 80% sure that's a Prugnat also, the short-point version of the previously-mentioned S4. I couldn't find any evidence like a catalog though, that's from memory. I have built frames with those, I just can't remember very authoritatively who made 'em.
The extra points on the back of the heat tube are put there by the framebuilder, a very common feature, on probably hundreds of brands of frames. Later, most lugs started coming with points there already. But most lugs without the points had enough 'meat' there to allow at least shortie points to be created, without welding on any extra metal, by grinding or filing off a bit to either side of where you want a point to appear.
