The first Team Raleigh frame that I saw in or around 1973 had these features ie the fluted sat-stay eyes and the gear-lever-boss bottle bosses. It was a 753 tubed model.
The top-eyes would not be difficult to make either by filing or easier still probably by milling. There was, after all, a fashion for double-fluted top-eyes in British frame-building circles dating from way back in the early 50s.
As for the botle bosses you would have thought that Gerald O'Donovan could have devised something a little more imaginative and less incongruous.
